Connie Davis is an internationally known  trainer, educator, speaker and consultant on health care design and health behavior change.  Connie has experience as a plenary speaker, collaborative director, improvement advisor, facilitator and workshop leader on chronic illness care design, care for older adults, rapid cycle quality improvement, self-management support, health literacy, patient-centered design, motivational interviewing and care coordination and management.  Connie also provides direct care as a gerontological nurse practitioner.  She is a member of the Motivational Interviewing Network of Trainers, the only professional organization that recognizes expertise in training in MI.


Connie is co-director of the Centre for Collaboration, Motivation, and Innovation, www.centreCMI.ca works to further its vision to improve health outcomes through helping people take active roles in self-managing their health.


Connie was a member of the team that created the Chronic Care Model (Wagner et al, 2001) and the clinical director of the national program, Improving Chronic Illness Care (USA, 1998-2005). Connie particularly enjoys working with clinicians and health care teams from rural and remote locations and the application of current evidence to the outpatient setting.
